The War Within introduced two gear upgrade currencies, Valorstones and Harbinger Crests. These currencies can be exchanged at the item upgrader NPCs, Cuzolth and Vaskarn.
Each item can be upgraded to its maximum level for a specific slot once. Upgrades to that slot will be cheaper following. The pity system, which was introduced in The War Within, also lowers the extreme upgrade variance.
